According to the report, both engines failed seconds after takeoff when the fuel cutoff switches unexpectedly shifted from ‘run’ to ‘cutoff’. The crash resulted in 270 fatalities. The cockpit voice recorder captured confusion between the pilots, with one questioning the shutdown while the other denied initiating it. Despite attempts to reignite the engines, the aircraft crashed 32 seconds after liftoff. The report finds no signs of sabotage, technical fault, poor weather, or significant bird activity. A 2018 FAA report had previously highlighted potential issues with fuel control switches in certain Boeing models.
